A template terms of service. Before publishing, adapt the provisions (prices, deadlines, cancellation rules) to your offer and consult a lawyer.
§ 1. General provisions
- These terms set out the rules for providing holistic counselling services by [IMIĘ I NAZWISKO], [KOD POCZTOWY, MIASTO] (hereinafter: the "Provider").

- A client may be a natural person with full legal capacity.
§ 2. Nature of services
- The services are of a wellbeing-supporting, relaxing and educational nature.
- The services are not medical services, psychotherapy or treatment and do not replace medical or psychological consultation.
- The client uses the services voluntarily and at their own responsibility, being aware of the above nature of the services.
§ 3. Arranging and holding meetings
- The meeting time is arranged individually – by phone, e-mail or via the contact form.
- Meetings take place in person or online, as agreed by the parties.
- The Provider may refuse or interrupt the service if its provision would be contrary to law, ethical principles or the client's wellbeing.
§ 4. Prices and payments
- Current service prices are given individually before the start of cooperation.
- Forms and deadlines of payment are set when arranging the service.
§ 5. Cancellations and rescheduling
- A cancellation or rescheduling of the meeting should take place at least [e.g. 24 hours] in advance.
- Rules regarding cancellations at shorter notice are agreed individually.
§ 6. Right of withdrawal (consumer)
- A consumer who has concluded a distance contract may in principle withdraw from it within 14 days without giving a reason.
- If the service is to be performed before this period expires, the client may consent to its start, acknowledging the loss of the right of withdrawal after full performance of the service.
§ 7. Complaints
- Complaints may be submitted to the e-mail address: info@is-m.de.
- The complaint should contain the client's details and a description of the objections.
- The Provider handles the complaint within 14 days of receipt.
§ 8. Protection of personal data
The rules for processing personal data are described in the Privacy policy.
§ 9. Final provisions
- In matters not regulated, the provisions of Polish law apply, in particular the Civil Code and the Consumer Rights Act.
- The Provider reserves the right to amend the terms; contracts already concluded are governed by the terms in force at the time of their conclusion.
