These Terms set out the rules for using the services and website of D-E-N-T-A-R-I-S Dental Clinic Sp. z o.o. (hereinafter: the "Clinic"). Using the Clinic's services means accepting these Terms.
The Clinic provides dental services including prophylaxis, conservative treatment, prosthetics, implantology, orthodontics and cosmetic dentistry. The detailed scope and guide prices are available under "Services".
The prices on the website are indicative. The final cost of treatment is set individually after consultation. The Clinic accepts payment in cash, by card and — for selected procedures — in instalments.
The patient has the right to information about their health, the proposed treatment and to give informed consent to the procedure. We uphold patient rights in accordance with applicable law.
Complaints about the services provided may be submitted in writing to the Clinic's address or by e-mail. We handle complaints within 14 days of receipt.
In matters not regulated by these Terms, the provisions of Polish law apply. The Clinic reserves the right to amend the Terms, of which it will inform on the website.
